Residential Services Activities Program Supported Living Supported Employment Transitions Behavioral Health Services Casemanagement

This program is designed around the unique needs of its participants. Individualized support is delivered in the person’s home and community integration, recreation and socialization. This program allows people to remain in their family home and attend local schools while receiving vital one on one support from Elmhurst staff to become as independent as possible.
