Residential Services Activities Program Supported Living Supported Employment Transitions Behavioral Health Services Casemanagement

This is our newest service - developed to provide support to adults (18 and over) who are living at home, not under public guardianship and not on the Medicaid waiver. The Elmhurst, Inc. case manager will coordinate services including housing, employment, medical/dental, any other therapies, recreation etc. They will also work closely with the individual and family members to identify what supports strategies can best be used to help reach the consumers goals. A primary goal of case management is to make sure that the individual is connected to the local community through volunteer opportunities, recreation, and job placement.
